-The completed DAF Military Athlete of the Year Application nomination form.
-Complete an up-to-date DAF Form 1206
-Submit completed packages to Marcel.Gibson@us.af.mil

The DAF Form 1206 is a template for sport-specific accomplishments.

All statements must be written in narrative form; paragraph formatting is acceptable.

Each header listed on the template must contain at least one statement; there is no maximum, but the content must not exceed the front page.
Use of acronyms must be used IAW Air Force Acronym and Abbreviation List and DAFMAN 36-2806, Military Awards, para 7.3.2 and 7.3.3.

Nominees MUST be:

All information regarding Award Eligibility is located in the Awards Program Guide under 1.3
-Active Duty, Guard, or Reserve
-Nominee who has PCSed within the current Award cycle (Check Awards Program Guide #1.3.7)
-Nominee must NOT be under investigation
-Nominee must NOT be under disciplinary actions within the award period

1.2 Awards Cycle
1.2.1. All awards are based on a calendar year award period. The A1 Awards Program inclusion dates for Legacy,
Individual and Force Support Capabilities Awards are from 1 January through 31 December.

1.2.2. Award Submission Timeline. Specific dates will be included in the nomination call.
November: Call for nominations
January: MAJCOM/Headquarters Air Force (HAF) selections
January: Award packages due to HQ AFPC
February: HQ AFPC award selection boards convene (virtually)
March: Site visits for LeMay and Eubank awards
May: Awards announced/message released
